Your Role

  • Spend quality time with patient’s to understand their goals, needs and desires to help allocate appropriate services to achieve this.
  • Help patients implement, organise and manage their NDIS plan by connecting participants with appropriate services that will help achieve their goals.
  • Ensure that participant’s and participant’s family / guardian are involved in the management and implementation of the NDIS plan.
  • Encouraging participant’s to manage their own plan to promote independence
  • Meet 1 Primary Care Services compliance requirements in relation to statutory WHS standards
  • Documentation and reports for each participant consultation

    What we require from you

  • Degree or cert / diploma in community services related to discipline
  • Current Working With Children Check / National Police Clearance
  • Current NSW Driver’s License (with own transport)
  • Preferable previous experience working in the disability sector, however applicants with no previous experience are also welcome to apply
  • Preferable First Aid Certificate
  • A compassionate and genuine attitude in helping those in need to achieve their goals.
  • Excellent communication and documentation skills
  • Self-motivated and committed to empowering individuals and promoting independence through teaching them skills.
