This is a Recruitment Pool for Fixed Term Part Time appointments at 30.4 hours per fortnight for up to 6 months with the possibility of extension(s).

We currently have eight (8) fixed term positions available.

This is an open-ended recruitment pool, and as such the following should be noted :

  • The panel will monitor this recruitment pool for applications, and will assess on a regular basis.
  • Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur.
  • Appointments may be made from this pool until 31 / 12 / 2020.
  • Position Profile : We are seeking to appoint suitably experienced Senior Social Workers to plan, manage and implement a Social Work service at an advanced practice level within General Practice and the community, utilising care coordination to the patient target group accepted into the program.

    Successful candidates will provide education and liaison to actively establish and build relationships with designated General Practices and other relevant services.

    Responsibilities include developing and implementing an evaluation process, recording, collecting and reporting key performance information and participating with development, embedding and future planning to ensure the program is evidence based and sustainable.

    General Practices participating in the program are located in the following areas :

  • North Perth
  • Quinns Rocks
  • Area Profile : The South Metropolitan Health Service (SMHS) delivers quality, safe and effective hospital and health services within a catchment area stretching more than 3300 square kilometres across the southern half of Perth.
