This is an open-ended recruitment pool, and as such the following should be noted :
Position Profile : We are seeking to appoint suitably experienced Senior Social Workers to plan, manage and implement a Social Work service at an advanced practice level within General Practice and the community, utilising care coordination to the patient target group accepted into the program.
Successful candidates will provide education and liaison to actively establish and build relationships with designated General Practices and other relevant services.
Responsibilities include developing and implementing an evaluation process, recording, collecting and reporting key performance information and participating with development, embedding and future planning to ensure the program is evidence based and sustainable.
General Practices participating in the program are located in the following areas :
