Allied Health Services are looking for an experienced Allied Health Practitioner - Advanced (Rehabilitation and Geriatrics) to provide high level clinical leadership across the multidisciplinary team in order to enhance the coordination of care delivered to rehabilitation and geriatric patients admitted to Redland Hospital.

About the Role
Provide high level clinical leadership across the multidisciplinary team in order to enhance the coordination of care delivered to rehabilitation and geriatric patients admitted to Redland Hospital.
This includes leading decision-making, in collaboration with the multidisciplinary team, in relation to admission to the inpatient rehabilitation program and the initial assessment and prioritisation of patients referred to the Geriatrician
The primary focus of this position is to provide overarching coordination of the shared care of all patients admitted to the rehabilitation program and geriatric evaluation and management service, in consultation with medical specialists and the other members of the multidisciplinary team.
This position works as an Advanced Allied Health Practitioner within a hospital rehabilitation and general medical ward setting.
This role may need to change and evolve over time to ensure it continues to meet current and emerging priorities. It has an advanced scope of practice that may progress to an extended scope of practice including shared and delegated responsibilities, as endorsed by the relevant professions across the multidisciplinary team.
