    The Opportunity

    A need for a proficient, organised and detailed-oriented Care Coordinator with proven leadership skills has now arisen to help continue LHI's high level of resident care.

    What do I do in this role?

    Reporting directly to the Care Manager of Residential Services, Glynde, the Care Coordinator will be a role model and leader in supporting and affirming the service excellence culture and values of LHI Residential Care Facility at Glynde site.

    You will oversee and supervise clinical governance ensuring our 120 residents receive high quality, holistic person-centred care from the nursing and carer teams.

    You will support these employees to meet best practises, directives and adhere to policies and procedures , particularly now in the current Covid-19 period.

    Attention to auditing and quality requirements are also key, as is an ability to demonstrate a thorough manner with all clinical documentation and reporting to ensure outcomes for consumers and stakeholders are met.

    A solid understanding and delivery of aged care Quality Standards, aligned with a safety focus at all times, form other parts of your responsibilities.

    A sense of collaboration, urgency and flexible approach are other qualities the successful Care Coordinator can bring for effectively working with the Care Manager and the team.

    Your assistance in mentoring, performance, and roster management will ensure your success in the role, and to meet care objectives for continuous improvement .

    What do I bring to be successful?...

  • A minimum of three years RN experience and Aged Care experience
  • Proven leadership skills
  • Registered Nurse with AHPRA
  • Passion for aged care
  • Great interpersonal skills for effective engagement with persons of diverse backgrounds, backed up by high written and verbal communication skills
  • Ability to be flexible, goal-oriented and work independently as required
  • Effective time management and prioritising skills
  • A can-do’ approach to pressures, competing demands and team matters
  • Ability to implement relevant LHI strategies, directives and procedures
  • Well-developed situational insight and responses
  • Sound computer skills in a broad range of applications including excel and word
  • Current and valid police check and Australian work entitlement
  • It would be fantastic if you had post-graduate nursing qualifications or management qualifications.

    About Our Client

    Lutheran Homes Inc. (now LHI Retirement Services) was founded in 1957 and they continue to grow today, striving to achieve aged care service excellence by valuing people.

    The village at Glynde consists of approximately 300 Independent Living Units, a Residential Care Facility, various community support facilities and a purpose-built church.

    The Hope Valley site enjoys similar facilities to those at Glynde. LHI cares for over 1,000 people and employs approximately 500 staff.

    LHI's caring and responsive management processes have ensured they have kept abreast of the challenges and expectations in the world of providing services to older Australians.
