Northern Sydney Home Nursing Service is actively committed to providing quality professional community nursing. The service utilised a progressive model of service delivery which promotes accessibility, patient autonomy and independence with measurable health outcomes.

What you'll be doing

Assess, plan, implement and evaluate community nursing care, in the patient home or clinic, in collaboration with individual / s and the greater health care team to achieve quality patient outcomes.

Providing nursing care to patients living at home.

Candidates will need to meet the following criteria :

  • Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment.
  • Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.

  • Registered Nurse Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) and relevant nursing experience.
  • Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.
  • Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.
  • Sound experience using electronic medical record systems and office applications, and evidence of personal time management capabilities and effective negotiation skills.
  • Current NSW Drivers Licence
